2020 Father of the Year Nomination Form

Nominations may come from a child, spouse, friend, YMCA staff member, or a fellow volunteer. Please complete the nomination form and personal essay honoring a dad who meets the following criteria:

Must be a Y member within the YMCA Dallas association
Nominees must have demonstrated the YMCA character values of Caring, Honesty, Respect, and Responsibility in their effors at the YMCA or their community.
